I am honored to be your coach and look forward to working with you to support your goals and the life you envision. As we begin our coaching relationship, I would like to present some important ideas regarding how we can best honor the process and each other. Please know that if you have questions or concerns regarding these, I am happy to discuss them with you at any time.

Your Coaching Plan
Career Pathfinder Plan with Career Assessments
Coaching Fee: $667 per month
Duration of Plan: 3 months
Included in your Plan:
- 9 one-hour coaching sessions
- Career Assessments to include Strengths, Personality Profile, and Occupational Interest Inventory.
- Career Results Summary
- Personal Development Plan
- Option for virtual check-ins (as arranged with your coach)
- Follow-up Session notes
- Text and email or private client log support between sessions (as arranged with your coach)
Your Private Client Portal:
As a Client, you will have access to your own private client portal. The portal is used to manage your client profile, complete online forms, store session notes, manage invoices and agreements, communicate with your coach, and access private pages and resources. You will be provided with login instructions and credentials for your portal. It is your responsibility to keep your credentials secure and restrict access to the portal from anyone other than you, the Client.
General Coaching Terms:
Attending Sessions: It's important to show up on time, be present (no multitasking) and uphold commitments to the coaching schedule. Your coach promises to do the same for you. The length of Sessions will vary depending on your choice of a coaching plan. Sessions may be held in-person, by virtual meeting, or by telephone as agreed upon with your coach. Check-ins are available between sessions by text, telephone, email, or the Virtual Support Lab as arranged.
Rescheduling and Canceling Sessions: You or your coach may need to reschedule or cancel your appointment. Please provide 24 – 48 hours of advance notice to accommodate changes. We will do the same for you! We understand last-minute situations may arise. Please email, call or text your coach immediately if you are unable to keep your appointment. We will do the same for you! Every attempt will be made to schedule a make-up session for a different time, the same week. All makeup sessions must be used within the timeframe of the billing period.
Missed Sessions: Your coach will be available to meet with you at the scheduled time. Sessions will close after fifteen minutes of waiting with a no-show or no word from the client. When missed, an attempt will be made to schedule a make-up or check-in session for a different time, the same week. Any unused missed sessions will be forfeited at the end of your billing period.
Payment for Services: Payment is due by check, money order, or credit card in advance of services rendered. All checks and money orders are to be made payable to: Center for Living Well with ADHD, and mailed or delivered to 3800 American Boulevard West, Ste 100, Bloomington, MN 55431. The client understands and agrees that Coach may suspend or terminate coaching services; without liability, for any client who does not meet the payment/financial terms of the agreement above.
Confidentiality and Privacy Policy: Your information at The Center remains strictly confidential and may not be shared to others unless: (1) You provide prior consent and authorization to release information; (2) In emergency situations or when there is a perceived risk of harm to self or others; (3) As required by a court of law; (4) Or to receive support from the Center and it’s employees on the Coach’s behalf.

With Coaching, all decisions in these areas are exclusively the responsibility of the client who acknowledges that decisions and actions as a result of coaching are their sole responsibility.
Amends to Coaching: At the Center, we work hard to ensure you receive the highest level of coaching support and services possible. If at any time, you are not satisfied with the direction of coaching, or life gets in the way of continuing your coaching, please advise your coach immediately in writing. Every effort will be made to support your needs and provide an amenable solution. Refunds for unused coaching sessions are not guaranteed and at the discretion of the coach, based on the situation and the terms of your coaching agreement if you wish to discontinue coaching early.
Discontinuation of Coaching and Refund Policy: When it is time to end our coaching relationship, it is your responsibility as a client to provide a minimum of 15 days notice which includes a paid, wrap-up coaching session. This gives Client and Coach time to capture learning and strategize for what is next in your life, as well as celebrate you successes!
